How do I know if my registration submitted online was received?
Each registered attendee will receive an email confirmation after their registration is completed.
How do I submit a purchase order for my registration costs?
Purchase Orders (PO) with accompanying check and registrations should be submitted by mail and postmarked by February 24th, 2012.
Can I still register online if using a Purchase Order?
No, if using a purchase order you must register by mail. Please note: The Registration table will NOT accept a PO or payment for a PO on the day of the conference.
Before I can process my PO, my company requires a W-9 form. How can I get one?

Scholarships:
Scholarships are offered through the sponsoring DOVIA’s to their members only.
Refunds:
Registration is non-refundable but can be transferred to a co-worker. Workshop selections and lunch choice must remain unchanged.
Accessibility:
All facilities are wheelchair accessible.
Food option:
Lunch and beverages are included in Thursday’s fee. A continental breakfast, hot lunch, and beverages are included in Friday’s fee. We will do our best to accommodate your dietary requirements within reason. Note your dietary requirements during registration.
Exhibitors/Vendors:

CEU’s:
Each participant will receive a certificate of attendance. This certificate may be used for the recertification activities required for your CVA. CEUs for social workers are not available.
